Thanks for your suggestion. It almost did the trick. I also had to comment out the following bit of the /etc/pcmcia/config.opts file to get it working:

# Extra port range for IBM Token Ring
#
#include port 0xa00-0xaff
#

Now, both the 2.2.14 kernel with pcmcia modules 3.1.8 and the 2.3.39 unstable linux kernel put the CEM33 modem on /dev/ttyS3, where it should be.
