It might be useful to see the kernel messages.  Also, what does
"cardctl status" report, right after booting with the card in?

One option (not really a fix, but a workaround) might be to play with
the PCMCIA bridge mode in your BIOS setup menu.  You should be able to
choose between three modes: "PCIC", "Auto", and "CardBus".  Try each
setting.  The "PCIC" mode should be ok, but will prevent you from
using 32-bit CardBus cards.  Which may not even be a concern.
